2024 Rabies Clinic
This form must be filled out and brought to the Free Rabies Clinic which will be held on Saturday, January 13, 2024 from 10:00am-12:00pm at the Public Works Garage located at 875 Summer St, Florence.
The rabies certificate will be mailed to you after the clinic. If you prefer email, please include your email address. If you need to renew your pet license, please include the fee (exact cash or check only) and we will mail the renewal & tag to you along with the rabies certificate.
NO LICENSES WILL BE ISSUED AT THE CLINIC.
License renewal fee: $10/dog $13/non-neutered dog; $5/cat
